Department of Health Center for Health Development in Region 9 has released the list of accepted applicants for Human Resource for Health Deployment Program 2019.
DOH has placed most of these health workers under Personnel Services-Contractual items for them to receive Magna Carta benefits, aside from the salary under the SSL 4th tranche. But there are still items under Job Order (Contract of Service), such as nurses as Public Health Associates.
This is for June to December 2019 deployment as ordered by DOH Central Office.

These are the requirements to be prepared, completed and brought to the Pre-deployment Orientation yet for rescheduling for those whose names will appear in the Lists for HRH Deployment Program June to December 31, 2019, both for those who will be hired under Personnel Services (PS) and those who will be hired under Contract of Service (COS) which will be posted in the PDOHOs within the week, hopefully:
-Three (3) sets of updated Personal Data Sheet duly accomplished (Form 212) for authentication by the Office;
-Original copy of valid Drug Test Result from a DOH-accredited Drug Testing Facility;
-Original copy of Nueropsychological Exam Result;
-Original copy of Medical Certificate;
-Three (2) sets of Original copy of authenticated PRC Rating (Blue Ink);
-Photocopy of updated PRC Card;
-Original copy of NBI Clearance;
-Two (2) copies of authenticated Transcript of Records;
-Two (2) copies of Statement of Assets and Liabilities and Networth (SALN);
-PAG-IBIG Enrollment Form with MID Number;
-Taxpayer’s Identification Number; and,
-PSA Copy of Birth Certificate.
For those who are new to the Deployment Program whose name is on the list, aside from the above requirements, the following pertain to necessary documents for ATM application which can be prepared to apply on your own and for PhilHealth enrollment for the Office to process:
1. Three (3) pcs. 1×1 recent photo;
2. PSA Copy of Birth Certificate and dependents, if any;
3. Photocopy of Marriage Certificate for married individuals;
4. Php 100.00 initial deposit for DBP ATM application;
5. Taxpayer’s Identification Number (TIN).
