DOH-13 opens application for HRH Deployment Program 2019

Department of Health – Center for Health Development (DOH-CHD XIII) in Caraga is now accepting application documents for the Human Resource for Health (HRH) Deployment Program for pooling purposes for the year 2019.

DOH HRH Deployment Program is an annual hiring and deployment of nurses, doctors, dentists, midwives, medical technologists, pharmacists and other allied health workers to geographically isolated and disadvantaged areas (GIDA) and other poor communities in the country.

Caraga, officially known as the Caraga Administrative Region or simply Caraga Region and designated as Region XIII, is an administrative region in the Philippines occupying the northeastern section of the island of Mindanao. The Caraga Region covers the following provinces: Agusan del Norte, Agusan del Sur, Dinagat Islands, Surigao del Norte, Surigao del Sur; Butuan City is the regional center.

DOH CHD XIII announces that submission of documents for the following positions shall be submitted from November 6-19, 2018 only:

1. Dentist II
2. Nurse II
3. Midwife II
4. Medical Technologist II
5. Pharmacist II
6. Project Development Assistant
7. Nutritionist-dietician II
8. Project Assistant II

AREAS OF DEPLOYMENT: Rural Health Unit, Birthing Facilities and Barangay Health Stations in Region XIII

Applicants may submit their applications from November 06, 2018 — November 19, 2018, 8:00 AM — 5:00 PM at Department of Health Center for Health Development Caraga – Human Resource and Development Unit, Pizarro St. Corner Narra Road, Butuan City.

Applicants who failed to meet the minimum Qualification Standards are disqualified. All applications submitted outside of the posted period will not be accepted.

A. Application Letter (Indicating PRC number)
B. Photocopy of updated PRC License 8, Certificate of Good Standing
C. Personal Data Sheet (PDS) CSC Form 212 – downloadable thru the Internet
D. Deployment Program/Project Application Form – downloadable thru the Internet
E. Photocopy of Diploma and Transcript of Records
F. Photocopy of Certificate of Employment (Current DOH employ. for 1 year or more need not submit)
G. Photocopy of Certificate of Trainings attend.
H. Photocopy of NSO/PSA Birth Certificate & NSO/PSA Marriage Certificate – if applicable
I. NBI Clearance 1. Hospital Clearance (Provincial DOH Office shall require hospital clearance for applicants currently engaged to public & private hospitals)

1. Use legal size or long bond paper
2. Submit the following requirements in a white folder with table of contents in tabbing sequence.
3. Place your FULL name at the right corner of the long white folder.

Currently hired HRH since January 2018 does not need to attach COE while HRH employed beyond January 2018 must submit COE of previous employer.

November 28, 2018 – Dentist II, Pharmacist II, Nutritionist – Dietician II, Pharmacy Assist.
November 29, 2018 – Nurse II (NDP) of Province of Dinagat Islands
November 29, 2018 – Nurse II (NDP) of Surigao del Sur
December 3, 2018 – Project Assistant II (PHA)
December 4, 2018 – Midwife II (RHMPP)
